Eine schwierige Aufgabe der extrazellulären Stimulation besteht darin, im äußerst dichten Nervengeflecht zwischen den gewünschten Neuronenpopulationen zu differenzieren und nur die gewünschten zu stimulieren. Mikroelektroden müssen an geeigneter Stelle implantiert werden und gleichsam entscheidend für die Selektion der Nervenzellen ist das Muster des ins Gewebe abgegebenen Stimulationsstromes.<br />Eine Anwendung, die Regulierung der verloren gegangenen Funktion der Harnsteuerung über die Blasenmuskulatur und den Schließmuskel bei durchtrenntem Rückenmark, wurde untersucht. Zu diesem Zweck werden die Neuronen in drei Kategorien unterschieden: die LC (local cells), das sind die Neuronen mit dem Zellkörper in der Nähe der Elektrode, die AT (axon terminals), das sind die Neuronen mit dem Zellkörper in mittelbarer Umgebung der Elektrode und die FOP (fibers of passage), das sind zufällig vorbeiführende Axone. Die LC repräsentieren die EUS (external urethral sphincter, Schließmuskel), die AT repräsentieren die BLA (bladder, Nervenzelle verantwortlich für die Blasenmuskulatur).<br />Die Untersuchungen sind mit einem in NEURON modellierten Nervenmodell vorgenommen, das im Anhang ersichtlich ist. Das Ziel ist es, nach dem Modell von Schwarz-Reid-Bostock-McIntyre-Grill (SRBMG) ein Motoneuron zu untersuchen und verschiedene Stimuli zu vergleichen.<br />Um eine gezielte Reizung der EUS oder BLA zu erreichen, wurden entlang des Neurons verschiedene Elektrodenpositionen auf anodische und kathodische Schwellenpotentiale untersucht. Anhand der Ergebnisse wird klar, dass das Neuron entlang des Axons sehr sensibel auf einen negativen Stimulus reagiert. Auf der dem Axon abgewandten Seite des Soma führen leichter positive Stimuli zu einem Reiz. Zu nah am Neuron konnte keine Differenzierung in EUS oder BLA erfolgen, da dieses vor allem in der Umgebung eines Ranvier'schen Schnürringes immer erregbar war. Zur Anwendung sollten symmetrische oder asymmetrische biphasische Puls-folgen kommen. Eine gewünschte Regulation der Ruhephase bei der Harnsteuerung wurde bei einem Elektrodenabstand ab ca. 200 Mikrometer von der Nervenzelle und einer asymmetrischen Pulsfolge mit Verzögerung gefunden.<br />

A challange in extracellular stimulation is to differentiate between the neuron populations and to excite just the desired ones.<br />Microelectrodes have to be implanted at the right place in the spinal cord and also the pattern of the stimulation current is important for the success.<br />An application, the regulation of the lost function of uretic control over the external urethral sphincter (EUS) and the bladder musculature (BLA, nerve to the bladder) in case of a damaged spinal cord, has been analyzed. For this purpose the neurons are divided into three categories: the LC (local cells), these are the neurons with the cell body not far from the electrode; the AT (axon terminals), these are the neurons with the cell body in middle distance of the electrode and the FOP (fibers of passage), these are the crossing axons with other functions. The LC represent the EUS and the AT represent the BLA.<br />For the studies a nerve model of a motoneuron has been designed with the simulation tool NEURON. The aim is to analyze the behaviour of this motoneuron after the SRBMG model (Schwarz-Reid-Bostock-McIntyre-Grill) and compare different stimuli.<br />To achieve a selected stimulation of the EUS and the BLA, different electrode positions along the neuron are studied on anodic and cathodic threshold potentials. Near the axon the neuron is easy to stimulate with negative currents (cathodic). On the side of the soma, which is on the opposite side of the axon, the situation changes. Here the neuron is very sensitive to positive currents (anodic). It was not possible to differentiate between EUS and BLA in close proximity of the neuron because of the Nodes of Ranvier that are very easy to stimulate.<br />In detail symmetric or asymmetric biphasic stimuli should be used. A desirable regulation of the resting state of the uretic control was found at an electrode to neuron distance over 200 micrometer and asymmetric biphasic stimuli with inter-delay.<br />
