Zusammenfassung

Einleitung: Ausgeprägte Dysgnathieformen des dentofazialen bzw.

dentoskelettalen Systems können einen bimaxillären Eingriff erforderlich machen, um ein sowohl funktionell als auch ästhetisch zufriedenstellendes Ergebnis zu erzielen. Soll das gewünschte Behandlungsziel durch eine intraoperative Rotation des maxillomandibulären Komplexes erreicht werden, so führt dies auch zu einer Veränderung der Okklusionsebenenneigung.

Ziel: Das Ziel dieser retrospektiven Studie war die Erfassung einer möglichen Veränderung der Okklusionsebeneneigung nach bimaxillären Eingriffen und ein eventuell damit in Zusammenhang stehendes erhöhtes Rezidivrisiko.

Methoden: Die retrospektive Vermessung der digitalen Fernröntgenseitbilder mittels OnyxCeph*3 Software von 54 bimaxillär operierten Patienten des SMZ-Ost Donauspitals Wien betraf Operationen der Jahre 2004, 2005 und 2006, nämlich jene die eine Kombination aus Le Fort I und BSSO mit oder ohne Genioplastik erhielten. Außerdem bedurfte es präoperativer, unmittelbar postoperativer, sowie 1-Jahr- und 5-Jahres postoperativer Fernröntgenseitbilder. Die kephalometrischen Messungen bestanden aus dem Winkel ANB, Wits appraisal, ODI, APDI, Winkelsumme im Björk'schen Polygon, Inklination der oberen und unteren Inzisivi, Overbite, Overjet. Weiters wurden die Angle Klasse und die Neigung der Okklusionsebene nach Downs, der anterioren und der posterioren Okklusionsebene zur Sella-Nasion-Linie und zur Frankfurter Horizontale zu den vier genannten Zeitpunkten erhoben.

Ergebnisse: Der präoperative, postoperative, 1 Jahr und 5 Jahre postoperative ANB-Winkel korrelierte signifikant mit dem PPAB-Wert (APDI) der 4 Erhebungszeitpunkte (r=-0.903, r= -0.628, r= -0.710, r= -0.701). Trotzdem stimmte die skelettale Klasseneinteilung nach dem ANB-Winkel nicht in allen Fällen mit jener des PPAB-Wertes (APDI) überein. Die postoperative Veränderung der Neigung der anterioren Okklusionsebene zur Frankfurter Horizontale war signifikant (p= 0,002), jene der posterioren Okklusionsebene zur Frankfurter Horizontale von präoperativ nach postoperativ war ebenfalls signifikant (p= 0,000).

Fünfzehn Patienten mit präoperativ skelettaler Klasse II (n=15 laut Winkel ANB, n=18 laut APDI) wiesen präoperativ neutrale, verminderte und erhöhte Neigungen der posterioren Okklusionsebene auf. Bei 12 bzw. 16 Klasse-II-Patienten (80 % laut ANB, 89 % laut APDI) blieb die Neigungscharakteristik der Okklusionsebene postoperativ unverändert. Für die Klasse-III-Patientengruppe (n=24 bzw. n= 28) bestand bei 3 (i.e. 13 % nach ANB bzw. 11 % laut APDI) präoperativ eine neutrale Neigungscharakteristik der posterioren Okklusionsebene. Bei 21 bzw. 25 Patienten mit Klasse III (88 % laut ANB bzw. 89 % laut APDI) lag präoperativ eine verminderte Neigung vor. In beiden Klassifizierungsgruppen erfuhren 8 der Klasse-III-Patienten eine postoperative Korrektur von einer verminderten in eine neutrale Neigungscharakteristik (33 % bzw. 29 %). Es bestand eine signifikante Wechselwirkung (p= 0,008) zwischen präoperativem ANB-Winkel und der zeitlichen Veränderung der Neigung der posterioren Okklusionsebene.

Hinsichtlich der postoperativen Stabilität des ANB- und PPAB-Wertes gab es zwischen jenen Patienten mit postoperativem Wechsel der Okklusionsebenenneigungscharakteristik und jenen ohne Wechsel keinen signifikanten Unterschied.

Schlussfolgerung: Diese retrospektive Studie zeigte keine Beeinflussung des Rezidivrisikos durch eine veränderte Okklusionsebenenneigung. Anhand der Daten besteht keine Notwendigkeit der Berücksichtigung dieser operationsbedingten Veränderung der Okklusionsebenenneigung in präoperativen Planungen. Jedoch sollte diese Erkenntnis durch weitere Studien untermauert werden.

Abstract

Introduction: The correction of pronounced dentofacial and dentoskeletal deformities by bimaxillary orthognathic surgery can affect the cant of the occlusal plane. Its extent of alteration was subject of this thesis.

Objective: This retrospective study measured differences between occlusal plane angulation before and after bimaxillary orthognathic surgery and the potentially increased risk of relapse due to changing the cant of the occlusal plane.

Methods: Using OnyxCeph*3 software, the digital lateral skull x-rays of 54 patients, who underwent bimaxillary surgery in the Vienna SMZ Ost hospital, were measured retrospectively. The patients operated in the years 2004, 2005 and 2006 and treated by combined Le Fort I and BSSO with or without genioplasty were implemented in this study, which analysed X-rays made preoperatively, immediately postoperatively, 1 year postoperatively and 5 years postoperatively. The cephalometric measurements comprised angle ANB, Wits appraisal, ODI, APDI (measured as angle PP-AB), angular sum in Björk's polygon, inclinations of maxillary and mandibular incisors, overbite, overjet, the Angle class and the cant of Downs'occlusal plane, the anterior and posterior occlusal planes referenced to the Sella-Nasion-line and the Frankfort Horizontal. All values were measured at the four time points.

Results: Preoperative, postoperative, 1 year and 5 years postoperative angles ANB and APDI showed significant correlations (r=-0.903, r= -0.628, r= -0.710, r= -0.701). However, the skeletal classification depending on angle ANB did not always confirm the classification depending on PPAB. Surgery significantly changed the angulation of the anterior occlusal plane to the Frankfort Horizontal (p= 0.002), and the posterior occlusal plane angulation to the Frankfort Horizontal (p= 0.000). Fifteen preoperative skeletal class II patients (according to angle ANB n= 15, according to APDI n= 18) showed neutral, low and high angulations of the posterior occlusal plane. Twelve and 16 preoperative skeletal class II patients (according to ANB 80 %, according to APDI 89 %) did not change its characteristic of angulation after surgery. Three (according to ANB 13 %, according to APDI 11%) of the preoperative skeletal class III patients (according to ANB n= 24, according to APDI n= 28) presented a preoperative neutral angulation of the posterior occlusal plane. Twenty-one and 25 preoperative skeletal class III patients (according to ANB 88 %, according to APDI 89 %) showed a preoperative low angulation of the posterior occlusal plane. In both classification groups 8 of the preoperative skeletal class III patients changed the characteristic of occlusal plane angulation from low to neutral after surgery. There was a significant interaction (p= 0.008) between the preoperative value of the angle ANB and the postoperative alteration of anterior and posterior plane angulation. There was no significant difference between the stability of postsurgical angle ANB and PPAB (APDI) of patients with or without postoperative change of the occlusal plane angulation.

Conclusions: In this retrospective study the surgical alteration of the occlusal plane angulation did not have any influence on the risk of relapse. On the basis of these results there is no need to overestimate the change of occlusal plane angulation in prospective preoperative plannings. However, further studies should support this finding.
