Zusammenfassung

Hintergrund

In der seitlichen Maxilla ist vor Einbringen von Implantaten oft ein Sinuslift erforderlich. Frühere Studien evaluierten mögliche Klassifikationen des Sinus maxillaris anhand dessen Morphologie, um chirurgische Interventionen präoperativ besser planen zu können und mögliche Komplikationen, sowie das Outcome besser einschätzen zu können.

Methodik

Die Datenbank der Universitätszahnklinik Wien wurde retrospektiv auf Patienten durchsucht, bei denen entweder alle Prämolaren (PM) und Molaren (M) (=’zahnlos’) oder 1-2 benachbarte PM oder M (=’teilbezahnt’) in einem Quadranten des Oberkiefers fehlten. Anhand von CT-Aufnahmen wurden Querschnittbilder (QB) im Zentrum der fehlenden Zähne angefertigt. Es wurden die Höhe, Fläche und Breite (2mm kranial des krestalsten Punktes und auf Höhe des Sinusbodens) des Alveolarkamms vermessen, sowie die Fläche und bukkopalatinale Breite des Sinus maxillaris an mehreren Messhöhen (2,4,6,8,10 mm). Anhand der bukkopalatinalen Breite des Sinus erfolgte eine Klassifizierung in eine enge, durchschnittliche und breite Sinusklasse. Zur Beurteilung des Outcomes eines Sinuslifts in Abhängigkeit von der Sinusklasse wurden Panoramaschichtaufnahmen vor Sinuslift, unmittelbar danach, sowie nach der Implantation herangezogen.

Resultate

Es wurden insgesamt 383 QB von 76 zahnlosen und 86 teilbezahnten Patienten (Altersdurchschnitt: 55,4 Jahre, 86 weibliche und 76 männliche Patienten; 58 PM1, 97 PM2, 129 M1 und 99 M2) vermessen. Die durchschnittliche bukkopalatinale Breite des Sinus maxillaris nahm mit ansteigender Messhöhe und nach distal gehend bis zum M1 zu. Sowohl bei zahnlosen Patienten im Vergleich zu Teilbezahnten, als auch bei Frauen im Vergleich zu Männern war die Breite des Alveolarkamms auf Höhe des Sinusbodens signifikant geringer. Des Weiteren korrelierte die Breite des Alveolarkamms auf Höhe des Sinusbodens mit der bukkopalatinalen Sinusbreite auf allen Messhöhen. Die Zuteilung der QB in eine schmale, durchschnittliche und breite Sinusklasse zeigte sowohl in den unterschiedlichen Regionen als auch Messhöhen eine hohe Variabilität. In über 50% der Fälle wechselte die Sinusklasse in den unterschiedlichen Regionen innerhalb eines Patienten sowie in den unterschiedlichen Messhöhen innerhalb einer Zahnregion zumindest einmal die Zuteilung. Zusätzlich waren in den Regionen PM1, PM2, M1 und M2 10,2, 13,4, 11,6 und 9,2% der QB nicht klassifizierbar und es wurde kein Zusammenhang zwischen der Sinusklasse und dem Outcome (=Reduktion der Augmentathöhe) eines lateralen Sinuslifts festgestellt.

Schlussfolgerung

Eine klinisch anwendbare Klassifikation für den Sinus maxillaris konnte aufgrund der Varianz seiner Morphologie nicht bestätigt werden. Jede Region sollte für sich bewertet werden, da eine Klassifizierung zu stark von Messhöhe und –region abhängig erscheint. Ein Zusammenhang zwischen der bukkopalatinalen Breite des Sinus maxillaris und dem Outcome eines lateralen Sinuslifts konnte nicht bestätigt werden.

Abstract

Background

In the posterior maxilla a sinus floor augmentation procedure is often required to allow implant installation. Previous studies evaluated possible classifications of the maxillary sinus to enable a better planning of the surgical intervention and to improve the estimate of possible complications and the outcome.

Methods

The records of the School of Dentistry in Vienna were screened for patients missing at one side of the maxilla either all premolars (PM) and molars (M) (= 'edentulous') or 1-2 neighbouring PM or M (= 'partially dentate'). By means of computed tomography scans cross-sectional images at the centre of the missing teeth have been collected. The height, area and width (i.e., 2 mm apically to the alveolar crest and at the height of the sinus floor) of the alveolar crest were measured as well as the area and bucco-palatal width of the maxillary sinus at several heights (i.e., 2,4,6,8,10 mm above the sinus floor). The average values of the bucco-palatal sinus width was used to establish a classification into a narrow, average, and wide sinus. Further, to assess the outcome of a sinus floor augmentation procedure in dependence of the sinus morphology panoramic radiographs before and after sinus lift, and after implant installation have been evaluated.

Results

Altogether, 383 cross-sectional images of 76 edentulous and 86 partially dentate patients (age average: 55.4 years, 86 female and 76 male patients, 58 PM1, 97 PM2, 129 M1 and 99 M2) have been evaluated. The average buccopalatal width of the maxillary sinus increased with increasing height of the sinus and distally until M1. The width of the alveolar crest at the height of the sinus floor was significantly lower for both edentulous and female patients compared to partially dentate and male patients, respectively. Further, the width of the alveolar crest at the height of the sinus floor correlated independent of the measuring height with the bucco-palatal sinus width. The allocation of the cross-sectional images into a narrow, average, and wide sinus class presented a high variability among different regions as well as measuring heights. In more than 50% of the cases, the sinus class varied at least once among the different regions within one patient as well as among the different measuring heights within one tooth site. In addition, at PM1, PM2, M1 and M2 10.2, 13.4, 11.6, and 9.2%, respectively, of the images were not classifiable and no correlation between the sinus morphology and the outcome of a sinus floor augmentation (i.e., reduction of augmentation height) was detected.

Conclusion

Due to the variance of the sinus morphology a clinically applicable classification cannot be recommended; i.e., since a classification appeared rather dependent of the measurement height and region, an evaluation of the sinus should be performed within each region separately. Further, no correlation between the bucco-palatal width of the maxillary sinus and the outcome of a sinus floor augmentation was detected.
